This page provides information regarding the referral process it is our aim to make the process simple to understand and easier for all concerned.
Our Basingstoke Residential Homes are registered for LD to comply with our registration you must :
- be over 18
- have a learning disability
Our Fareham based Residential Care home is registered for MH to comply with our registration you must :
- be over 18
- have a mental health diagnosis
Bliss accepts referrals on a planned and emergency basis, however all referrals must be assessed and funding approved prior to admission to our homes.
Referrals come via telephone, comtact form or email from:
Social Workers / care Managers
Social workers or Care managers would visit the home(s) to view facilities and meet staff to decide if home can meet the individual's needs.
Initial discussion would determine if referral process would continue:
Care Manager sends health care assessment, OT reports and other relevant documentation.
Assessment booked by Bliss staff to meet 'referred individual' at current placement to gather information from 'individual's current care staff or relatives.
- Transition visits arranged
- Assessment report sent to care manager detailing our assessment offering a placement with fee quotes for assessed support package or letter sent explaining why an offer can not be made at this stage,
